This request must contain the following information: your name and surname, address for the purposes of notifications, photocopy of your National Identity Document or Passport, and request in which the request is specified. In the case of representation, you must prove it through reliable document. To send this request you can use the models made available by the Spanish Agency for Data Protection, which can be signed electronically.
